Cytokine profile of aqueous humour of anterior chamber in patients with age-related macular degeneration

Abstract Details

PURPOSE:To study the change in the local immunological profile in neovascular membrane on the background of wet age-related macular degeneration.

The levels of cytokines were determined in aqueous humour of anterior chamber of the eye in patients with wet age-related macular degeneration. 22 patients with wet age-related macular degeneration were examined in order to determine content of cytokines. The patients were divided into 3 groups depending on the morphological features of neovascular membrane: the 1st group - 8 (8 eyes) - patients with classic neovascular membrane; the 2nd - 7 (7 eyes) - patients with mixed neovascular membrane; the 3rd - 7 (7 eyes) - patients with active fibrovascular membrane. Taking of aqueous humour of anterior chamber was performed during intravitreal injections by insulin syringe 30 G through the paracentesis of paralimbal area of cornea. The results were recorded using «Multiscan» automatic photometer at the 450 nm wavelength. The results were expressed in pg /ml. Normal level of these cytokines in the aqueous humour of anterior chamber is for interleukin-1β - 1,94 pg / ml; IL-6 - 4,5 pg / ml; for IL -18 - 364 pg / ml; VEGF - 60 pg / ml, PEDF - 3,08 pg / ml.


The level of IL-1β in the aqueous humour of the anterior chamber with classic neovascular membrane (group 1) averaged 1,422 ± 0,23 pg / ml. Concentration of IL-1β was higher and amounted to an average of 2,667 ± 0,789 pg / ml (r1-2 <0.05) during the forming a mixed choroidal neovascularization (CNV) (group 2). The reduction was observed in the level of IL-1β in patients of group 3 to 1,327 ± 0,306 pg / ml. In group 1, IL-6 level averaged 2,346 ± 0,756 pg / ml. In the case of aqueous humour leakage under the retinal pigment epithelium (RPE) (Group 2) the concentration of interleukin was 4,674 ± 1,87 pg / ml (р1-2<0,05), in the 3d group - 1,79 ± 0,872 pg / ml. The concentration of IL-18 in the aqueous humour of anterior chamber during the formation of classic CNV (group 1) corresponded to an average of 373,313 ± 93,452 pg / ml. Its concentration was 320,874 ± 47,251 pg / ml and 337,483 ± 45,233 pg / ml in the 2nd and 3rd groups respectively.


VEGF concentration in aqueous humour of the anterior chamber in the patients of group 1 was on average at 722,141 ± 48,479 pg / ml, the formation of mixed CNV - 353,37 ± 20,819 pg / ml (р1-2<0,05). Fibrovascular membrane component developed on the background of low concentration of VEGF in the anterior chamber of the eye (the 3d group) - an average of 120,216 ± 24,074 pg / ml (р1-2, р1-3<0,05). Local level PEDF averaged 0,756 ± 0,313 pg / ml with classic neovascular membrane (Group 1), with the development of a mixed variant (Group 2) - 0,549 ± 0,114 pg / ml, with the development of fibrosis (Group 3) - 0,531 ± 0,082 pg / ml. Changes in the level of cytokines (IL-1β, IL-6, L-18, VEGF) in aqueous humour of the anterior chamber indicate interaction between degree of process activity and location of neovascular membrane.
