To show that sometimes simple acts can save sight in such dramatic condition.

Clinical case report of 70 years old women with an acute onset (30 mn) of unilateral inferior absolute scotoma due to retinal arterial branch embolism. Slit lamp retinography (SLD7 DC3 Topcon) showed the sitting of the embolus in vascular bifurcation.
After anterior chamber paracentesis to reduce intraocular pressure associated with genupectoral position to increase arterial retinal perfusion pressure, the arterial retinal embolism disrupt within 10 minutes.
Anterior chamber paracentesis with genupectoral position should be always attempted in emergency care of retinal embolism.
