Purpose:

To find out whether uneventful phacoemulsification induces any change in peripapillary retinal nerve fiber layer (RNFL) in patients without co-existent ocular and systemic diseases

Setting:

Pamukkale University, Department of Ophthalmology, Denizli, Turkey

Methods:

The patients who underwent uneventful phacoemulsification for cataract were included in this prospective study. The patients with any systemic diseases as well as ocular diseases that may affect RNFL and macula or prevent optimum imaging were excluded. Peripapillary RNFL was measured by optical coherence tomography (Heidelberg Engineering GmbH, Heidelberg, Germany). The RNFL analysis was performed on 6 sectors including the nasal, temporal, superonasal, superotemporal, inferonasal and inferotemporal sectors and the mean RNFL. The measurements were performed preoperatively and at the postoperative 1, 3 and 6 month visits.

Results:

A total of 12 patients were included in the study. The mean age was 62.58±10.12 with 5 females and 7 males. Preoperative RNFL thickness in superotemporal, superonasal, temporal, nasal, inferotemporal and inferonasal sectors and the mean RNFL were 128.3±12.6, 109.3±18.8, 69±10.2, 73.9±9.9, 135.4±25.8, 118.5±17.6 and 96.7±5.5 respectively. RNFL thickness in superotemporal, superonasal, temporal, nasal, inferotemporal and inferonasal sectors and the mean RNFL were 126.8±13.3, 108.9±16.9, 78.4±35.1, 76.2±9.3, 137.1±25.8, 121.5±19.2 and 98±6.2 respectively at the postoperative first month. RNFL thickness in superotemporal, superonasal, temporal, nasal, inferotemporal and inferonasal sectors and the mean RNFL were 127.6±11.2, 109.5±19.9, 69.5±9.2, 76.2±10.8, 136.5±25.2, 121.3±18.8 and 97.9±6.2 respectively at the postoperative third month. RNFL thickness in superotemporal, superonasal, temporal, nasal, inferotemporal and inferonasal sectors and the mean RNFL were 126.6±10, 108.7±16.8, 69.6±9.1, 75.5±9.7, 135.6±24.5, 119.5±17.6 and 97±5.4 respectively at the postoperative sixth month. The change in RNFL thickness in inferonasal sector at the first month and in nasal sector at the third month compared to preoperative values was significant (p=0.046 and p=0.050 respectively).

Conclusions:

There was a temporary change in RNFL thickness which was significant in inferonasal sector at the first month and in nasal sector at the third month after surgery. At the sixth month, RNFL thickness was similar to the preoperative values in all sectors.
