to evaluate the effects of triamcinolone acetonide as monotherapy of serous retinal pigment epithelium detachment (PED) associated with age-related macular degeneration (AMD)

44 patients (50 eyes) with serous PED associated with age-related macular degeneration were observed. 19 patients (21 eyes) were injected 0.1 ml (4 mg) of triamcinolone acetonide (TA) intravitreal, 25 patients (29 eyes) received 1.0 ml (40 mg) of TA to subtenon space. Patients underwent visiometry, tonometry, fluorescein angiography (FAG), optical coherence tomography (OCT). OCT examined the following structural characteristics: height and length of PED. Follow up examinations were carried out before treatment and at the 1st, 3rd, 6th, 12th, 15th, 18th, 24th, 32nd months of follow up.
The mean BCVA of patients before intravitreal TA was 0.3 (SD; 0.27). The range of serous PED height was 115 to 1200 microns, the length being 850 to 4660 microns. At the 32nd month visual acuity increased to 0.35 (SD; 0.25) (р=0.3). The height of the serous PED was from 125 to 580 µm and the length of serous PED was 400 - 1655 microns. Before subtenon TA average visual acuity of patients was 0.4 (SD; 0.33). The height of the serous PED was in the range from 105 to 770 µm, the length was from 410 to 4545 µm. At the 32th visual acuity increased to 0.42 (SD; 0.34) (р=0.3). The height of PED was 80 - 495 µm, the length was 225 - 2020 µm. During the whole observation period out of 50 cases of TA application, complete reattachment of PED was observed in 30%. No retinal pigment epithelium tear was noted. After the 30th month, in 8% were observed the formation of choroidal neovascularization.
Application of TA permits to decrease the height and extension of serous PED within 32nd months in most of cases. Reattachment of the RPE observed in 30% of cases. Visual acuity remains stable in all cases. TA may be useful for preserving of anatomycal and functional of the retina in patients with serous PED associated with AMD. At the 32nd months of observation in 8% of cases development of choroidal neovascularization. RPE tear formation was absent.
