To evaluate the effect of treatment of hypotony with trabecular meshwork photocoagulation.

Retrospectively analyze IOP change of 16 hypotony eyes, or about to be hypotonic after silicon oil removal, after trabecular meshwork photocoagulation. Green or red laser was applied to destroy the 360 degree trabecular meshwork 1-4 times (1.8 times in average) under the gonioscope or endoscope. Nine cases exposed a lot of RPE (large retinal defect), 1 case had ciliary membrane, 4 cases had ciliary membrane and retinal defect at the same time, 1 case had ciliary detachment, 1 case was chronic uveitis.


IOP of 16 eyes was 3.5-11mmHg (7.73mmHg in average before laser), 13 cases of them had intraocular silicone oil. After laser, the IOP was 6-18mmHg, (11.55 mmHg in average at the end of 1 week to 2 years fellow-up). 13 eyes’ IOP increased, 7 of their IOP <10 mmHg before laser and ≥10mmHg after laser; 2 cases had the same IOP, and 1 case lost of fellow-up. Six cases removed silicone oil, and none had retinal detachment or hypotony after surgery, during the follow-up.


Trabecular meshwork photocoagulation can effectively increase the intraocular pressure, provide opportunity for removal of silicon oil despite of large area of retinal defect.
