To determine intraocular pressure (IOP) fluctuation effect of cataract surgery beyond the ultrasound energy to the macula.

36 eyes of 33 subjects underwent low energy phacoemulsification. The macular thickness and volume were examined by spectral domain optical coherence tomography (SD OCT). Impact and correlation between low effective phaco time (EPT) and macular thickness and volume changes were measured to ensure pure IOP fluctuation effect.
There is no correlation between EPT and macular thickness and volume changes, P=0.12 (P>0.05). In the paracentral macular area, the thickness of three quadrants significantly increased (superior P=0.012; temporal P=0.001; and nasal P=0.014). Peripheral macular thickness also increased significantly in the superior (P=0.008) and temporal macular areas (P=0.003). The macular volume increased significantly after phacoemulsification (P=0.001).
Beyond the ultrasound energy, IOP fluctuation may affect the delicate structure of the macula in same way with the various laws of physics.
