Abstract Details

Purpose:

To study the intra operative retinal macular morphology during macular surgery using hand held spectral domain optical coherence tomography (SD OCT)

Setting:

a tertiary referral hospital based.

Methods:

A hand held SD OCT device was used to image and characterize the pre operative and intra operative images in patients undergoing vitrectomy for macular pathologies. The intra operative imaging was performed after the internal limiting membrane peel.

Results:

intra operative imaging from 10 patients undergoing macular hole surgery were analyzed. The minimum diameter decreased in all cases, range 23 to 120 µ, mean 65 µ. (p=0.038). An area of increased sub retinal hyporeflectivity was noted at the level of the RPE after the ILM peel. The pre existing Inner segment /Outer segment junction disruption did not increase after the surgical manipulation. The cystoid changes near the hole edge reduced significantly after the ILM peeling. 5 patients undergoing epi retinal membrane removal were also imaged. The central retinal thickness showed variable response in these patients . Significant sub retinal hyporeflectivity was seen after the peel. Increased corrugation of the retinal surface seen after completion of the peel.

Conclusions:

The intra operative use of hand held SD OCT provides a novel method of visualizing and quantifying changes in retinal anatomy during macular surgery
