Purpose:

To illustrate the role of Schlieren effect in detecting the posterior break in a case of myopic macular degeneration with retinal detachment

Setting:

Retina clinic of a tertiary eye care centre

Methods:

A high myope (-22D) male with a macula off inferior retinal detachment with myopic macular degeneration and posterior staphyloma underwent pars plana vitrectomy. The break was not identified during fundus evaluation. After PVD induction and complete vitrectomy, suction alone was used at the posterior pole to look for egress of subretinal fluid. The sudden appreance of schlieren, helped to locate the break nasal to the disc with pigment clump around the retinal break. The retina was settled by fluid air exchange by draining the subretinal through the break and silicone oil injected after barrage laser.

Results:

The schlieren effect in a rhegmatogenous retinal detachment with a posterior break in the staphyloma helps to identify the break. The retina remianed attached under silicone at 4 months of follow up.
