Evaluation of fundus findings in eyes with Purtscher’s Syndrome

A 61 year old man had both distortion and decrease in vision 2 days after chest compression and broken leg during the traffic accident. He present to ophthalmology clinic 3 weeks after the accident. In clinical examination VA measurement, OCT and FFA evaluations were performed.
VA was 0.3 in effected right eye while it was 10/10 in other eye. Fundus examination revealed multiple white retinal patches combined with focal superficial retinal haemorrhages mainly located at superior part in temporal arc. OCT showed diffuse edema on the maculae-papillary bundle, hyper reflectivity in the inner retinal layers and macular elevation. FFA showed that hypofluorescence patches corresponding areas to retinal whitening and haemorrhages. Close clinical observation preferred before any treatment. Two and a half week later his VA increased to 8/10 and visual distortion disappeared. On fundus evaluation retinal haemorrhages on inferior part of temporal arc disappeared while still present on superior part. On OCT edema of maculae retinal bundle decreased and macular elevation disappeared.
Significant spontaneous improvement of findings and symptoms could be observed in a short period after the unset in Purtscher’s Syndrome. Visual acuity improvement and recovery of the symptoms may related to recovery of macular elevation.
