To present a case of retinal artery macroaneurysm treated with intravitreal aflibercept injection.

We present a 89-year-old woman with decreased visual acuity in her left eye. Ophthalmoscopy, fluorescein angiography and optical coherence tomography imaging was performed to make the diagnosis.
Visual acuities were 20/25 in the right and 20/80 in the left eye. Examination in the right eye was unremarkable. Ophthalmoscopy for the left eye showed exudation in the macula and retinal artery macroaneurysm with surrounding haemorrhage in the infero-temporal vascular arcade. Fluorescein angiography demonstrated dye leakage in the late phases consistent with macroaneurysm. And optical coherence tomography revealed serous macular detachment with hyperreflective dots in the macula. Intravitreal aflibercept injection was administered as treatment and anatomical, functional improvement was achieved after one dose of injection. Visual acuity increased to 20/25 and no other complications were observed in follow-up period.
Injection of intravitreal aflibercept should be considered as an alternative treatment choice for retinal artery macroaneurysm. It assists rapid resolution of macular edema and regression of macroaneurysm.
