Analysis of topographic changes in choroidal thickness after intravitreal aflibercept for exudative age-related macular degeneration

Purpose:

To investigate topographic changes in choroidal thickness after 3 monthly loading doses of intravitreal aflibercept in exudative age-related macular degeneration (AMD) patients by implementing the choroidal thickness map for more comprehensive analysis of choroidal structural change.

Setting:

Retrospective, interventional case series.

Methods:

Treatment-naïve exudative AMD patients who received 3 monthly loading doses of intravitreal aflibercept were included. EDI raster scans consisting of 31 B-scans were performed using spectral-domain OCT at baseline and 1 month after the third loading injection. The automated segmentation lines delineating the ILM and bottom of the RPE-Bruch's membrane complex were re-aligned to fit the bottom of the RPE-Bruch's membrane complex and chorio-scleral interface, The choroidal thickness was automatically calculated with built-in software and a topographic choroidal thickness map was implemented in 9 subfields of the ETDRS grid. The differences in average choroidal thickness in the 9 subfields before and after treatment were analyzed.

Results:

A total of 41 eyes of 41 patients were included. Patient average age was 73.2 years and 70.7% were male. Average best-corrected visual acuity and central foveal retinal thickness showed significant improvement from 0.85±0.59 LogMAR and 472.4±141.9 µm at baseline to 0.66±0.54 LogMAR and 273.5±77.3 µm 1 month after the third loading dose. Average choroidal thickness in the centre 1 mm circle decreased from 245.9±130.1 µm to 225.5±123.4 µm (p=0.001) and significant choroidal thickness reduction was observed for all the 9 subfields with the exception of the 6 mm ring inferior subfield (p=0.128). An average of 17 µm in choroidal thickness reduction was observed in the 9 subfields.

Conclusions:

Homogeneous significant reduction in choroidal thickness was observed after 3 loading doses of intravitreal aflibercept for exudative age-related macular degeneration on the implemented choroidal thickness map. Further studies are warranted to investigate the clinical significance of more comprehensive choroidal structural analysis as done herein.
