Combined per fluro carbon and silicone oil tamponade vitrectomy in inferior retinal detachment in primary vitrectomy

Abstract Details

Purpose:

To assess the efficacy of perfluoro carbon (PFCL) with silicone oil as a short term tamponade in eyes with acute inferior retinal detachment

Setting:

In a tertiary care retina centre

Methods:

40 patients who underwent primary vitrectomy for inferior rhegmatogenous retinal detachment duration less then 2 weeks with inferior breaks only were put on a combined perfluoro carbon and 1000 centistoke silicon oil for a duration of 3 weeks till the laser or cryo takes its effect, patient is asked to strictly maintain an erect position at all times to give a good support of the PFCL in order to prevent vigorous movement of the PFCL within the globe the upper portion is occupied by silicone oil, patients were followed up on day 1, day 7, day 20 and visual acuity, intraocaular pressure, OCT and indirect ophthamoscoly was performed at every visit, second surgery for PFCL oil removal was done after 3 weeks and patients were assesed for vision, IOP, anatomical and visual success for up to 6 months duration

Results:

35 eyes out of 40 eyes( 88.5%) showed succesful anatomical attachment after removal of tamponade from the eye and maitained the status even at 6 months, 5 eyes out of 40 did show retachment and underwent resurgery using external buckle and revitrectomy and oil combination (conventional) of which 1 required 2 additonal re surgeries, visual acuity imporved by more then 3 lines on logmar testing in (30/35) 85.7 %, 5 patients showed less then 3 line improvement, 5 patients showed minimal rise in iop in early post op period but were easily controlled with topical antiglaucoma drugs, OCT measurements did not show significant change in retinal thickness at macula

Conclusions:

Short-term combination of pfcl with oil can be considered as an effective option especially in patients who defer or cannot to maintain prone position, early emulisificsation of oil, bonding of oil PFCL requires different approach in removal, early removal of tamponade to prevent toxicity to retina is important
