Effect of intravitreal injection of dexamethasone implant (Ozurdex®®) on corneal endothelium in retinal vein occlusion

Abstract Details

Purpose:

To evaluate the effects of intravitreal dexamethasone implant (Ozurdex®®) on corneal endothelium in patients with retinal vein oclusion

Setting:

The study included 22 eyes of 22 patients who had retinal vein occlusion with macular edeme

Methods:

Full ophtalmolojical examination of patients were performed during each visit. We measured endothelyalial cell densıty (ECD), coefficient of variation of cell size (CV) and hexagonalıty by corneal specular microscopy at baseline, 1 month and 3 months after intravitreal injections. Central Corneal Thickness (CCT) was measured by corneal specular mıcroscopy ( Konan®, Japan) and topography (CSO Sırıus 3D Scheımpflug Camera®,İtaly). Corneal endothelium was evaluated with corneal specular mıcroscopy.

Results:

The mean central corneal thickness(CCT) was measured 536,8 μm at baseline, 536,8 μm at post-one month and 538,2 μm at post-three months by topography (p>0.05). The mean CCT was measured 567,5 μm at baseline, 564,1 μm at post-one month, 556,5 μm post-three moths by speculer microscopy (p>0.05). Mean endothelial cell densıty was measured 2211,7 cells/mm2 at baseline, 2207,1 cells/mm2 at post-one month and 2163,8 cells/mm2 post-three months in the injected eyes (p=0,018). Mean ECD was measured 2219,8 cells/mm2 at baseline,2265,9 cells/mm2 at post-one month and 2102,8 cells/mm2 post-three months in the uninjected eyes (p=0,179) . Coefficient of variation (CV) was measured 37,86 at baseline, 40,14 at post-one month, 40,05 at post-three months in the injected eyes (p=0,511). Coefficient of variation (CV) was measured 40,70 at baseline, 41,35 at post-one month, 41,94 at post-three months in the uninjected eyes (p=0,842). Hexagonalıty was measured 54,86% at baseline , 55,81% at post-one month , 56,63% at post-three months in the injected eyes (p=0,481) . Hexagonalıty was measured 53,70% at baseline , 53,95% at post-one month , 53,72% at post-three months in the uninjected eyes (p=0,879).

Conclusions:

There was no statistically sıgnifıcant difference in mean CCT in all time points (P>0.05). The mean ECD was decreased significantlly 3 months after the injections compared to baseline measurements in the injected eyes (p=0,013). There was no sıgnificant differences in mean CV, percentage of hexagonality and pachymetry in all time points during three months in the injected and uninjected fellow eyes (P>0.05). In this study we showed that intravitreal dexametasone implants may cause a reduction in endothelial cell density without causing significant change in endothelial cell morphology. So we must be careful about effect of intravitreal dexsametasone implant on corneal endothelium.
