Intravitreal triamcinolone for non-arteritic anterior ischemic optic neuropathy
    Purpose: To evaluate the efficacy of 4 mg of intravitreal triamcinolone (IVTA) in nonarteritic anterior ischemic optic neuropathy (NAION). Settings: Institutional. Methods: A prospective, interventional case series. Four eyes of four patients with severe visual loss due to NAION were treated with 4 mg IVTA (study group). The control group consisted of 6 consecutive patients with NAION who received no treatment. Patients were evaluated by Early Treatment Diabetic Retinopathy Study’s (ETDRS) visual acuity and visual field measurements and fluorescein angiography. The results of both groups were compared. All patients were followed-up for at least 9 months. Results: In the study group, the mean visual improvement was 4, 5.8 and 6.2 lines at the first and third week and final visit, respectively. Optic disc swelling markedly decreased at the first week and totally disappeared at the third week examination in all eyes. In the control group, the mean visual improvement was 0, 0.7 and 1.3 lines at the first and third week and final visit, respectively. All the control eyes showed resolution of the optic disc swelling between the fourth week and third month visits. No marked change in visual field defects was observed in both groups. Conclusions: IVTA provides rapid resolution of optic disc swelling with no marked visual field improvement in patients with early stages of NAION. Further studies are needed to see whether IVTA has beneficial effect on visual acuity in NAION.
