Vitreous surgery for central retinal artery occlusion
Purpose: Central Retinal Artery Occlusion (CRAO) usually causes abrupt and irreversible visual loss. Although multiple therapeutic regimens have been proposed for it, the effect seemed to be limited. In this study, vitreous surgery with vessel cannulation and thrombus disruption as a new technique for the treatment of CRAO was investigated. Setting: Retrospective, noncomparative, consecutive case series. Methods: Eight patients with CRAO were enrolled. BCVA was measured, fundus photograph was taken. For some patients, FFA was also taken on admission, at 48 hours, 1 week and 2 months after intervension. Results: Circulation was restores immedietly in 4 cases, gradually after operation in 2 cases. In 2 cases, there was no change, among which CRVO occurred in one case 20 days later. No other complications occurred. At two months, visual acuity had improved for two or more lines in 4 cases (50%). VA remained the same in the rest of the cases. Conclusions: Vitreous surgery with vessel cannulation and thrombus disruption seems to be an effective and safe treatment for CRAO.
