Recurrent retinal detachment due to persistent myopic macular hole
Purpose: To study the intraoperative findings in cases with recurrent retinal detachment due to myopic macular hole and to find the causes for recurrence. Highly myopic eyes are often complicated by macular hole retinal detachment (RD). The pathogenesis of myopic macular hole with RD remains poorly understood. A hypothesis that tangential traction exerted by epiretinal membranes (ERM) contributes to myopic macular hole is suggested. Methods: Nine eyes of recurrent retinal detachment due to macular hole and high myopia were studied. All patients underwent previous pars plana vitrectomy (PPV) and silicone oil with recurrence of detachment either under silicone or after silicone oil removal due to persistence and reopening of the hole. Results: Pre-macular traction in the form of residual vitreous cortex or membranes was found and removed in all cases. Postoperative OCT showed closure of the hole in 8 cases. Conclusions: Pre-macular traction could be the cause of the development and recurrence of myopic macular hole retinal detachment.
