The ocular toxocariasis
Methods: Patient (male, 50 y.o.) applied with complains of a stain in front of right eye (OD) which appeared one day ago. Two years ago he was treated for chorioretinitis in the left eye (OS) of unclear aetiology. Visual acuity (VA) was - OD=0.9, VA OS=0.2. Patient was examined by ophthalmoscopy, OCT and FAG. Those examinations showed: OD - only the high detachment of RPE, OS - the old areas of chorioretinal atrophy and hyperpigmentive scars. For treatment corticosteroids were used. Results: After 5 days of treatment by corticosteroids VA OD decreased to 0.4. Diode laser transpupillary thermotherapy was applied. The detachment of RPE disappeared, but VA OD didn't increase. In view of frequently visits of the patient to Tailand, he received consultation in the institute of tropical parasitology. The increase of Toxocaral fluorescent antibody was found. Treatment was conducted for toxocariasis. After 1.5 month of treatment VA OD was 0.9-1.0, VA OS=0.2. Now, after two years VA OU are stable. The fundus examination showed the hyperreflective and slightly hyperpigmentive area in macula in right eye without symptoms of inflammatory. Conclusions: The diagnosis of toxocariasis can be made only by identifying the larva histologically. But positive Toxocaral fluorescent antibody tests, frequently visits to Tailand and good results from anthelmintic treatment allow to make the definite diagnosis - the isolated ocular toxocariasis.
